You'll see when you finish the game...it will ask you if you want to start a new game and it will be the only time you can access the elite mode.  The only problem is that it will erase all your items and equipment when you start it.  If you start a new game in the same mode all of your items and equipment will transfer.

Hi Liquid, If I remember correctly...Beat it on easy upgrading all your stuff.  By the time you finish the game you wont have everything upgraded it will take you into a chapter or two of your second play through on easy to get them all.  BUT, make sure you save the game right before the last boss fight, VERY IMPORTANT...you can only access the hardest mode (insane mode?) once you beat the game. Once you got every trophy except for the one gun one...go back to your saved game before the boss battle, beat the game then start Insane mode.  You wont have many problems just using the plasma cutter fully loaded to beat the game on insane mode.   Got it?
